Request for Proposals Homeless Outreach Coordinator (two positions) Blue Ridge Area Coalition for the Homeless (BRACH) Funding Period: July 1, 2026 – June 30, 2027 The Blue Ridge Area Coalition for the Homeless (BRACH) is seeking proposals from qualified organizations to provide Homeless Outreach Coordinator services for the VA-504 region. BRACH is the Continuum of Care Lead Agency for the Greater Charlottesville area and is a community‑based homeless services non-profit dedicated to homelessness prevention, emergency shelter coordination, transitional housing pathways, permanent supportive housing, and data‑driven community collaboration to end homelessness across the region. Project Overview The Homeless Outreach Coordinator will support outreach efforts throughout the community, strengthen connections to housing stability resources, and participate in regional efforts aimed at reducing chronic homelessness. This role will focus on engaging unsheltered individuals, coordinating with local partners, and connecting people to shelter, rapid rehousing programs, and supportive services. Scope of Work Proposals should outline the applicant’s experience and approach related to: Conducting outreach to individuals experiencing street homelessness Coordinating with local agencies, emergency shelters, and community partners Facilitating access to affordable housing and low‑income housing support resources Supporting data collection for regional homelessness data‑driven solutions Providing documentation and communication for case management activities Ensuring culturally competent and trauma‑informed service delivery Outreach case conferencing: How outreach case conferencing meetings will be organized and facilitated Communication methods between outreach teams and partner agencies Processes for follow‑ups, shared case responsibilities, and coordinated care Experience with report submission Funding Funding for two Homeless Outreach Coordinator positions is provided by the Blue Ridge Area Coalition for the Homeless for a one‑year term beginning July 1, 2026 and ending June 30, 2027.
